Admissions Checklist
Must be 16 or older
Background check. Admission will be denied: If convicted or found guilty of abuse, neglect, or mistreatment of elderly.
If convicted or found guilty of misappropriation of a resident's property.LA Nurse Aide Registry - Admission will be denied if individual has a finding on the LA Nurse Aide Registry. (Included in background check).
Direct Service Worker (DSW) Registry Admission will be denied if individual has a finding on the DSW Registry. (Included in background check).
National Sex Offender Registry (nsopw.gov). Admission will be denied if individual is included on the National Sex Offender Registry. (Included in background check).
Negative TB skin test or CXR (within 12 months). Blood test accepted.
Drug test.
Flu vaccination or waiver if during flu season.
Enrollment Process
1. Submit $45 application fee and complete online application (upon completion you will be contacted with further instructions). Please confirm class dates prior to completion.
2. Complete background check ($45) online.
3. Submit proof of negative TB skin test or chest x-ray from within past 12 months, negative drug test, flu vaccination or waiver if during flu season.
4. Pay $150 registration fee.
